General improvements and bug fixes

  • Created new sub-permission which allows non verification owners to forward verification. Existing assurance manager permission group will automatically be updated to include this new sub-permission.
  • Updated verification results dashlet to exclude verification with N/A result.
  • Removed pagination for verification summary dashlet.
  • Updated risk controls grid view to include site comments and site tag IDs.
  • Updated the risk cause to include the qualitative likelihood value in the bowtie model even when the bowtie is in semi-quantitative mode.
  • Fixed an issue where the verification template details are not shown as filterable options in the verification activity timeline dashlet.
  • Fixed an issue where controls by effectiveness dashlet drill-down is not working.
  • Fixed an issue where the cause group layers are not shown in the bowtie model if there are hidden nodes within the bowtie.
  • Fixed an issue where the bowtie PDF printer is causing the browser to crash when headers and footers are hidden.
  • Fixed an issue where duplication of master bowtie is creating an ad hoc bowtie instead.
  • Fixed an issue where the inherent likelihood is returning as 0 when there is no preventative risk control.

Note: As we are getting very close to the next major update (Version 8.0), one of the major changes that we will be implementing is simplifying the user management. We will soon be introducing the usage of email address as the username within RiskView and the present username will be retiring in the next major update. Users with SSO enabled will not be affected by this change.

Updated Master and Site Bowtie Review Wizard

  • With the latest update, users will have more controls over their site bowties, including the ability to:
    • Assign risk control owners.
    • Assign tag ID for risk controls.
    • Allocate department and location for the bowtie.
    • Update the risk rating for bowtie (only applicable for bowties in simple mode).
  • The latest update also includes some improvements in the overall review process:
    • Restyled the related cause, consequence and controls pages:
      • These pages now have separate segments for each cause/consequence for easier navigation, especially for a large bowtie.
      • The comments from the related master bowtie will also be shown in the cause, consequence and controls sections.
    • Restyled the summary page:
      • Users will now have better visibility of the bowtie in the summary page, where they can set to see rejected nodes and hide away all controls.
    • Updating the master bowtie will now show the affected registers at the beginning of the page.
    • Admin users now have the ability to set all site bowties as active upon assigning a register type to a register.

General improvements and bug fixes

  • Updated verification summary dashlet to now include the register prefix in the register name.
  • Updated assurance report to exclude inactive MAEs and draft bowtie data.
    Changed the font colour of bowtie print preview to black for better visibility.
  • Fixed an issue where the actions by status dashlet is showing duplicate actions when drilling down the dashlet.
  • Fixed an issue where the change in project name not updating the project risk register view.
  • Fixed an issue where cost benefit analysis is not correctly shown in the risk control.
  • Fixed an issue where the edit button on the bowtie perspective not showing properly when browser zoom is less than 100%.
  • Fixed an issue where the imported values (risknode code) for risk studies not showing correctly on the screen.

  • Implemented support for duplicating risk studies across register.
  • Improved in-tray loading time.

  • Fixed an issue where bowties cannot be duplicated from the risk scenario grid view when the MAE cell is selected.
  • Fixed an issue where bowtie print preview does not show all bowtie components when cause group overlay is enabled.
  • Fixed an issue where the maximum initial risk rating for a risk study is not updated upon deleting the risk data underneath.

Changes in this release include:

Tightened Security

  • Implemented enforcement of password complexity for account.
  • Implemented account lockout due to multiple failed login.
  • Implemented improvements in other areas, such as disabling frameable response, improved prevention against account harvesting within Meercat RiskView.

Verification Activities Mobile App

  • Improved overall performance when loading the list of verifications within the app.
  • Improved the verification list layout within the app.
  • Improved the synchronisation process between the data within app and the server. The app now keeps a log for any synchronisation process and is accessible from the side menu.
  • Reduced the push notifications sent out by the app. The app will now group all notifications based on the verification collection.
  • Fixed an issue where the app behaves abnormally when the current logged in user credential details have changed.
  • Fixed an issue where the cached data is being cleared upon logging in with a different user.
  • Fixed an issue where the user is unable to navigate back to the verification list after going through the update connection details option.

General improvements and bug fixes

  • Implemented a zoom in feature during a mouse hover in a large bowtie. The details of the focused node will be shown at 100% to the user.
  • Included new permission type which allows non-admin users to edit/create master bowtie.
  • Included related MAE details in the performance standard details report.
  • Added support for enabling/disabling email notifications for the following modules: bowties, lessons learned and verification activities. Those options are enabled by default.
  • Added support for filtering functionality (showing/hiding rejected nodes) within site bowtie model.
  • Added support for adding attachments while an action is underway.
  • Updated verification results dashlet so that the results are grouped by the performance standards if any of the axes is performance standard.
  • Improved loading performance for verification results dashlet.
  • Fixed an issue where the overlapping tooltips within the risk rating map dashlet are preventing users from seeing the information.
  • Fixed an issue where the monthly verification activities dashlet is showing incorrect results when drilling down into the dashlet.
  • Fixed an issue where pressing the tab key within the web verification activity wizard causing graphical glitches.
  • Fixed an issue where the risk rating map dashlet is not using the terminology set in the configuration.
  • Fixed an issue where the related user is duplicated upon duplicating an action.

General improvements and bug fixes

  • Added support for severity reduction on mitigating risk controls.
  • Added support for multiple output frequency calculation for intermediate event. The new system policy can be found under Configuration Editor > Register Policies > Analysis > Enable Output Calculation Method On Intermediate Event, with the following options being available:
    • OR – multiple incoming frequency values will be added together for the output (the default)
    • AND – multiple incoming frequency values will be multiplied together for the output
  • Added support for filtering items on site bowtie model.
  • Updated verification result dashlet to have options where user can pick the latest (the default) or average verification result to show.
  • Updated risk rating map dashlet to have initial risk rating as source.
  • Optimised performance for risk scenario details report, actions report and performance standard audit result report.
  • Fixed an issue where configuration file cannot be saved.
  • Fixed an issue with reset password functionality.
7.12.1 Release Notes

Changes in this release include:

Verification Module

  • Hierarchical view in verification wizard:
    • The new hierarchical view in the navigation menu replaces the previous display of question number.
      • It is structured by performance standards at the top level, followed by the performance elements and then the performance criteria which reflect the individual questions.
    • The updated menu also allows user to filter the questions based on the status of each question as well as the contents (i.e. with action/attachment).
  • Verification initiation in mobile app:
    • Verification initiation allows mobile app users to create ad-hoc verification activities on the go, whether to verify base controls or major accident events.
    • Individual users can create verification activities without relying on the system administrator to create and manage the activities.
  • Verification collection in mobile app:
    • Instead of showing verification activities as a flat list in the mobile app, mobile app users now have the option to show them as verification collections.
    • Verification collections provide an intuitive way of grouping verification activities generated from the same template at the same time.
    • Verification collections assist in providing a structured overview of verification activities and better allow RiskView users to maintain an overview of large-scale projects and operations through grouping associated verification activities.
  • Verification template has been updated:
    • To allow creation of critical control verification based on the register type filter specified in the template.
      • The performance criteria will be included in the generated verification activity where:
        • The performance criteria’s parent element has no register type specified or matches one of the specified register types.
        • The register’s register type matches one of the specified register types.
        • The base control is used by any bowtie within the selected register.
    • To allow creation of critical control verification per matching risk control name within the specified register.
  • Improvements for verification wizard:
    • Mobile app now supports sites with single sign on enabled.
    • Fixed an issue where pressing the back button on the mobile app will exit the app, instead of navigating to the previous page.
    • Fixed an issue where related bowtie section is returning bowtie data where risk control is not critical for critical control verification.
    • Fixed an issue where the title of the verification activity is not shown when coming from email hyperlink.
    • Fixed an issue where users can’t click on the hyperlink in the introduction section.
    • Fixed an issue where users can’t download the attachments if the verification activity is in read-only mode.
    • Added search support for drop-down fields (i.e. action department).
    • Included related bowtie data for verification activities via bowtie review and risk scenarios (via major accident event).
    • Removed redundant headers.
    • Replaced the red asterisks for invalid field with red bolded headers.
    • Replaced submit/summary page with submit confirmation page.

General improvements and bug fixes

  • Updated RiskView outgoing email to noreply@meercat.com.au.
  • Updated risk rating map dashlet to have multiple sources:
    • Highest consequence severity & likelihood (existing method).
    • Top event risk rating.
    • Maximum consequence risk rating.
  • Added support for enabling/disabling email notification for action module. This option can be found under Register Policies > Notifications > Notification for Action Module.
  • RiskView now logs the last user who applied the configuration change.
  • Fixed an issue where action notification gets sent out twice if the action owner is either the action implementer or the action approver.
  • Fixed an issue where risk studies can’t be duplicated.
  • Fixed an issue where severity and likelihood values are not using the default values in the bowtie mini properties panel.
  • Fixed an issue where new nodes are not added in site bowties upon publication of master bowtie.
  • Fixed an issue where bowtie reporting data is not available for site bowtie on initialisation.
  • Fixed an issue where the verification results crosstab dashlet showing incorrect data if dashboard has 2 or more with the same parameters.
  • Fixed an issue where verification results crosstab dashlet showing incorrect data for verification groups, base controls, major accident events and performance standards.
  • Fixed an issue where top 10 global items dashlet not showing items with score less than 0.05.

Changes in this release include:

Register Type

  • A new global item called “Register Type” is created and is accessible from Administration > Register Types.
  • Register types are designed to categorise risk registers and to be able to link master bowties to a register type rather than in individual register. Register types will allow rapid deployment of master bowties to new registers that reflect the same situation as another.
  • Register types are essentially categories that allowing users to further segment their risk registers and they are defined by the system administrator(s).

  • RiskView Mobile App: Fixed an issue where action participants are not saved back to the server.

7.10.2 Release Notes

When: All environments will be updated on Saturday, 2nd June 2018 at 08.30 AWST.

Changes in this release include:

Master Bowtie

  • A new global item called “Master Bowtie” is created and is accessible from Risk Studies > Master Bowties.
  • Master bowties can be used to create site bowties, and site bowties will automatically be updated upon publishing the master bowtie updates.
  • Master bowties are excluded from all risk rating dashlets and reports and they can only be edited by admin users.
    • Editing master bowtie is similar to editing normal bowties, with the exception that the admin user needs to publish the changes once he/she is done.
    • Upon publishing the changes of master bowties, all subscribing site bowties will be updated and status set to Awaiting Review.
    • Site bowtie owners will receive email notifications upon publication of master bowtie updates. If none is specified, then the email notifications will be sent to the Register owner of the subscribed site bowties.
    • Admin users can also opt to publish existing bowtie as master bowtie (this option is available from the toolbar).
  • Site bowties are mirror images of master bowtie:
    • Upon creating a site bowtie, user has to choose a master bowtie to copy from.
    • All related causes and the preventative controls will be shown to the user and all selected by default.
      • For any deselected item, user has to provide a mandatory comment to why that item isn’t selected for the site bowtie.
      • User can also put in optional comment for selected item.
      • The same workflow will apply for consequence and the mitigative controls.
    • On confirmation, user has to fill in the review history for the creation of site bowtie.
    • The following restrictions will apply to site bowtie:
      • User can only accept/reject items from the master bowtie and user will not be able to create new item in the site bowtie.
      • User can only create action and allocate bowtie owner in the site bowtie. All other functionalities are locked down and this includes changing the bowtie analysis type, updating the risk control effectiveness.
    • User can choose to unlink site bowtie from master bowtie.
      • Upon unlinking a site bowtie, automatic updates from the master bowtie will no longer occur.

Verification Activities

  • Native application is now available in Apple iOS and Google Android platforms. Users can now perform their verification activities on their mobile phones using the native application, with both online and offline capabilities supported. Find out more about our new mobile app here.
  • The web browser interface has also been updated to match with the design of the mobile app, with improvement in performance and reduced loading time.
  • Support for web browser offline functionality will be dropped in 7.11 release and all offline verification activities are to be performed through the mobile app in version 7.11 onwards.

  • A new report called Risk Scenario Actions Report is created which would show actions with linkages to the controls and their related cause or consequence pathways.
  • Included action notes in the Actions Report.
  • Increased precision to 16 decimal places.
  • Added bowtie status in the risk control grid views.
  • Added support for attachments and actions in causes and consequences.
  • Added support for customising the prefix texts used in the bowties review verification type. The prefix texts can be found under the Configuration Editor > Register Policies > Auditing > Bowties Review Question Prefix.
  • Fixed an issue where the date field will always get set back to 1 day prior.
  • Fixed an issue where the Monthly Verification Activities dashlet is not reporting any data.
  • Fixed an issue where drilling down on Verification Results dashlet on MAE column/row is showing unrelated data.
  • Fixed an issue where enabling event is not included in the report if it is directly connected to the consequence or top event node.
  • Fixed an issue where inactive bowties are shown in the Assurance Report and base control names are reported incorrectly.
  • Fixed an issue where comments are repeated for enabling events in the Risk Scenario Details Report.
  • Fixed formatting issue for the action sections in the Risk Scenario Details Report.
  • Fixed an issue where the maximum initial and residual risk do not get cleared out when there is no cause in the inner grid in the risk study worksheet.
  • Fixed an issue where the Verification Results dashlet is showing incorrect data if there is filtering on columns/rows.
  • Fixed an issue where action created in the study worksheet will appear in the next row.
  • Fixed an issue with slow query on Verification Results dashlet.
  • Fixed an issue where action status does not get set to In-Progress after submitting a verification activity.
  • Fixed an issue where the Top 10 Global Items dashlet is not reporting the correct usage count for base controls.

7.9.1 Release Notes

When: All environments will be updated on Saturday, 18th November 2017 at 08.30 AWST.

Changes in this release include:

Verification Group

  • A new global item called “Verification Group” is created which can be optionally attached to Base Controls and/or Risk Controls.
  • A new column called “Verification Group” is created to the Risk Controls and Base Controls Views.
  • A new report called Controls by Verification Group is created which would enable grouping and filtering of related Risk Controls and Base Controls for each Verification Group.
  • A new Verification Type called “Critical Controls by Verification Group” is created within a Verification Template so that by picking a Verification Group (and optionally, an MAE), a user could generate verifications for all Risk or Base Controls that are connected to that Verification Group (and optionally, to bowties corresponding to that MAE).
  • Verification Results dashlet has also been updated to include verification data involving Verification Group.

General improvements and bug fixes

  • Improvements on working in the bowtie model:
    • Implemented the ability to remove one node from a pathway by selecting it and doing a Shift + Click and drag
    • Implemented the ability to copy one or more nodes by selecting and doing a CTRL + Click and drag
  • Improvements to the Verification Wizards:
    • Made the main question (previously known as Performance Criteria) to be more prominent than the other supporting information
    • Made the Verification name to be visible at all time, even when on mobile devices
    • Changes are automatically saved back to the server (when there is internet connectivity) upon switching pages
    • Fixed an issue where scroll position is carried over to the next question
    • Fixed issue with table formatting in the Related Bowties table
  • Implemented the ability to apply additional filter on Risk Register Worksheet report. The default option for the Risk Register Worksheet report is to show all risks, and users can now filter on monitored risks. Users can enable monitoring from the bowtie’s properties panel.
  • Implemented the ability to generate Verification Activity based on related Base Control or Risk Scenario for the Critical Control Verification. Users can now choose to generate 1 Verification per matching Base Control or Bowtie.
  • Implemented two new reports:
    • Assurance Report: this report shows all Critical Base Controls that are linked to MAEs (through connections in Risk Controls and Risk Scenarios), as well as all related verification results, and all actions derived from those verification activities
    • Controls by Verification Group: this report shows all Risk Controls and Base Controls that are connected to Verification Group
  • Implemented a new functionality that allows user to see related Verification data from the bowtie model. Read more about it here.
  • Implemented a new register policy that allows user to override the Risk Control criticality even when Base Control is attached and assessment is done at the Base Control level. The new register policy can be found under Register Policies -> Controls -> Risk Control Criticality Override.
  • Added Base Control Tag ID in the Risk Control grid view.
  • Fixed an issue where project cannot be deleted from the grid view.
  • Fixed an issue where filters get reset for grid related dashlets (i.e. Verification Results dashlet).
  • Fixed an issue where headers for the Risk Rating Map dashlet show incorrect values when the X axis is severity and Y axis is likelihood.
  • Fixed an issue where the duplicate functionality has been disabled for the following grid view:
    • Verification Template
    • Draft Risk Scenarios
  • Fixed an issue where the details for other participants are not showing up in the Risk Study report.
  • Fixed an issue where users cannot log out if SSO is enabled for their instance.
  • Fixed an issue where the Verification name is not shown in the Verification Wizard title on rare occasion.

7.9 Release Notes

When: All environments will be updated on Saturday, 9th September 2017 at 08.30 AWST.

Changes in this release include:

Verification Module

  • Verification template:
    • New verification type – bowties review. This option uses critical risk control as the questions for the verification, and will create a verification for each bowtie found that exceeds the risk rating threshold set in the template.
    • Included a new option which upon completion of a verification, allows user to update related base control or MAE for the “As Defined in this Verification Template” option.
    • The register picker now shows hierarchically.
    • Performance standard type added to verification template which allows users to filter results for the following dashlets:
      • Verification activity timeline
      • Verification summary
  • Verification activity timeline:
    • New option to send reminder email to verification owner.
  • Monthly verification activities:
    • Data is now shown in line chart, which allows data from previous months to be shown as trend indicator.
    • Drill down report will be shown upon clicking on the cell.

Dashboards and Dashlets

  • User can now exit the dashboard page when it is in edit mode. Warning will be shown if there are unsaved changes when trying to exit the dashboard page.
  • Implementation of a new dashlet, Risk Rating Map.
    • This dashlet gathers bowtie information from the register and plots it on the graph which uses the likelihood and severity range from the system configuration.
    • User can specify the likelihood and severity range for the dashlet.
    • This dashlet also supports grouping based on:
      • Register
      • Owner
      • MAE
  • Reworked the following dashlets:
    • Actions by status – this dashlet is now shown as stacked columns, with ability to show where the origin of the action is.
    • Risk consequence count – this dashlet is now showing the qualitative severity values in the secondary axis.
    • Controls by effectiveness – this dashlet now supports filter based on critical/non-critical control, and the verification status, whether it is verified and not overdue, verified and overdue or not verified.
  • Implementation of consequence categories filter for the following dashlets:
    • Risk count matrix
    • Risk consequence count
    • Top 10 register items
    • Top 10 global items
    • Bowtie trends
    • Risk rating map

User Experience Improvement

  • Account management:
    • Users can now request for their password to be reset.
    • This step will require user to enter username, and if user can’t remember the username, he can also request the username to be sent to his email address.
    • With this change, it is important that all RiskView users must have valid email address. This will also tie in with our roadmap, where we will be using email address as the username in the future release.
    • Users can also change their password within the application (under “My Profile” option).
  • Better visibility of the application when it is unavailable:
    • On rare occasion, system admins might want to update the system configuration, which will then invoke a system recalculation.
    • During this system recalculation, the application will not be available (and the webpage will not respond) to anyone until it is over.
    • With this change, the application now shows the recalculation progress when users try to navigate to the application while it is not available.
  • Implemented better “Find” functionality within the system:
    • Users can now choose the scope of the search based on:
      • The register
      • Item type
    • Users can also choose to group the search results based on register or item types.

General improvements and bug fixes

  • Implemented the ability to run report when there is no entry in the grid view.
  • Implemented the ability to overwrite report date from the report viewer.
  • Actions now show where the origin is and user can choose whether to show it in the grid view (it is hidden by default) by enabling this newly added register policy, Action Show Source Fields In Grids.
  • Guidewords, parameters and deviation templates have been converted to global entities and all existing data will be remapped so that they appear in all registers.
  • The active view list will be reduced to 5 upon browser refresh. This is done to reduce the load on the browser.
  • Added Base Control column in the study worksheet.

Reworked Control Assurance

One of the feedbacks we received from users regarding the control assurance module is that it can get quite confusing and intimidating to conduct the assurance activity (e.g. completing the inspection checklist). We take that as a challenge and have since been working on a new user interface to overcome this issue, so that the overall process is more intuitive to the users. You can read more about the Verification Process here.

The changes in control assurance include:

  • Restructured Verification Template
    • Rearranged fields so they appear in a more logical order, as well as adding hints to all editable fields.
    • New functionalities:
      • New field that supports HTML text formatting – Introduction. The contents of this field will be used in the first page of the Verification Wizard.
      • New verification type – As Defined in Verification Template. This option allows users to create Verification Wizards without any item specification (such as Base Control, MAE).
      • New register scope – Select Registers from List. This new option now allows users to create Verification Wizards in selected Registers.
      • New option – Performance Standard Source. This new option allows users to select which Performance Standards to use when generating the contents of the Verification Wizards.
      • Criteria Context now supports multiple selection.
  • New approach to complete the assurance activity using the new interface, the Verification Wizard
    • The previous version utilises a standard worksheet style to complete the assurance activity, which can be quite confusing to new users.
    • The new Verification Wizard utilises a step by step process, which is more intuitive to new and existing users.
    • The new Verification Wizard can adapt to different devices, such as desktop, laptop with small screen resolutions, mobile devices, tablets.
    • Invalid questions will be marked accordingly and users can switch between devices easily at any time during the Verification.
  • Updated email notification to include:
    • Option to access the print preview of Verification Wizard.
    • An ICS file (calendar file) attachment which users can save to their email client calendar.
  • Changes to Verification Activity Timeline dashlet:
    • New option to access the print preview of Verification Wizard.
    • Score will no longer be displayed until Verification is completed.
    • Removal of Scope indicator as it is no longer relevant.
  • Updated control assurance related reports:
    • Audit Activity Criteria Responses – This report shows related Verification Results and Responses based on the chosen Performance Standards and Verification Templates.

    • With the changes to those two reports, users can now click through in those reports, to show additional Verification details.

User Responsibilities System

One of our goals in making the user experience better is easier data management. As the system grows for a company, users may often find old data that no longer meets the current requirement.

With this new update, we are introducing a new data governance framework called Responsibilities. The User Responsibilities System consists of a collection of queries that flag issues where specific system policies are not being complied with.

Each Responsibility tests a single condition, and when the condition is being met, no issue is raised. When the condition is not met, then an issue is raised with the person closest to the issue in the ownership tree.

This new feature also includes a reworked In-Tray which will show all the issues related to the currently logged-in User.

  • Prior to this release, when a user linked a Cause to a Cause Group, the likelihood values from the Cause Group were imposed on the linked Cause.
  • With this release, user can now specify whether or not those values are imposed. This option is accessible from the Configuration Editor, navigate to Register Policies -> Analysis -> Causal Group Allow Likelihood.
    XXL Bowties

    • How many nodes would you expect in a bowtie?
      • RiskView has now been enhanced to handle up to 3,000 nodes per bowtie. While we know that not many of you will be getting there any time soon, there was a limit and it has now been worked through and optimised.
  • Configuration Editor
    • Search Functionality
      • As there are more than 100 options within RiskView that are configurable, sometimes it can be time consuming to find the correct options that you want to configure from the Configuration Editor, so with this release, users can search from within the Configuration Editor and make changes from the search results. This powerful functionality not only allows you to find all the configuration elements related to one item, but allows you to edit them in-situ, dramatically reducing the effort required.
    • Selective Recalculation
      • Every time the configuration is altered a certain amount of time is required to recalculate all the values according to the new configuration values. In systems with thousands of bowties, this can take a significant amount of time. As of this release, recalculation time is significantly improved thanks to a number of performance enhancements, not the least of which is the selective recalculation based on the changes performed.
  • Implementation for calculation of Potential Loss of Life (PLL)
    • With this release, users can now calculate the Potential Loss of Life for a consequence
      • To enable it, navigate to the Configuration Editor, Register Policies -> Consequences -> Show PLL Calculation In Consequences. This option will only be visible when the bowtie is in Semi-Quantitative mode and PLL Worksheet is enabled within each Consequence.
    • RiskView supports two modes for calculating the PLL:
      • Simple PLL Method
        • Users will be asked to input the probability of involvement and fatality. Based on those inputs, the fatality rate will be calculated and it will be used in conjunction with the input of average exposed population to get the potential loss of life
      • Fatality Distribution Method
        • Users will be asked to input the fatality distribution and the PLL will be calculated based on the fatality distribution and the score behind each distribution scale
    • The Risk Scenario Details report has also been updated to include PLL details on Consequences when enabled for a particular bowtie
